Project Overview: The HIV Center and the Nanjing College
for Population Program Management are conducting a demonstration
project in Kunshan County, Jiangsu Province, to test a dual
protection (DP) intervention promoting concurrent protection
from unintended pregnancy and HIV and other STDs within
family planning (FP) services. It emphasizes AIDS and STD
education and counseling and recommends the use of a barrier
contraceptive, such as the male or female condom, either
alone or in conjunction with a non-barrier contraceptive.
The intervention is an adaptation of a successful family
planning service provider training program and counseling
strategy that was implemented by the Association for Reproductive
and Family Health in six Nigerian family planning clinics
through World AIDS Foundation funding in 1999.
The specific aims of the Nanjing Project are to (1) adapt
the DP training program for family planning providers and
the client education and counseling intervention implemented
in Nigeria for use in Chinese FP service settings; (2) train
10 Master Trainers (Nanjing College and Kunshan County staff)
in DP programming through a Train the Trainers Program;
(3) assist the 10 Master Trainers to train 40-60 health
care providers in DP education and counseling in the Kunshan
County FP Service Center and in all township and village-level
FP service sites (N=40) in one township; and (4) evaluate
the DP training and pilot test and evaluate the adapted
intervention in selected FP service sites, evaluating both
its feasibility and short-term effectiveness in increasing
condom use among FP clients and their partners.
As part of the pre-intervention formative research, nine
focus groups and 40 in-depth interviews have been conducted
with FP clients, married male partners of FP clients, and
FP service providers. In addition, we conducted 206 observations
of FP client-provider interactions and exit interviews with
204 FP clients. Seventy FP providers completed a self-administered
questionnaire related to the provision of FP services and
their HIV/AIDS-related knowledge and attitudes. After being
trained in a five-day Train the Trainers Workshop in DP
Education and Counseling was conducted, the Master Trainers
then trained 57 Kunshan County family planning service providers.
The counseling intervention with FP clients is underway
and the evaluation of its results will be available in August
2006.
