Project was created by the NYS Department of Health-AIDS Institute as part of New York State’s (NYS) Ending the Epidemic 2020 and HRSA’s Special Projects of National Significance (SPNS) initiative.The project is designed to facilitate the timely entry of young HIV+ individuals age 18-34 into HIV care, encourage vulnerable young persons to remain in care, and sustain viral load suppression among those in care.
